What to have ready
- The home's name and address, and its 9-digit facility number if you have it (it's on every listing here).
- What happened, when, and who was involved.
- Your contact details, unless you want to stay anonymous.
Every licensed home must post these contacts where residents can see them. To read a home's past inspections and complaints, see our guide on checking a license and inspection reports.
